The "&" was removed from the game's title, likely to make it sound less like a certain tabletop RPG. The subtitle was also redrawn and made all-caps, and both it and the main title had their palettes changed. The font was changed to a more traditional one, and the menu was put on a separate screen to make room for the expanded copyright text.
Enemies
The medusa enemies are topless in the Japanese version.

See the Ending
In the Japanese version, pressing Left + Right on Controller 2 during the introduction will jump straight to the ending. It's impossible to press Left and Right at the same time on a normal controller, so this would only work with modified controllers.
